Step 1:
Consult your doctor or nurse practitioner
Schedule an appointment with your doctor to discuss your symptoms and why you feel medical cannabis is a viable alternative to conventional medications and therapies.

Step 2:
Have your doctor complete the Delta 9 Medical Document
Your healthcare practitioner must fill out and sign the Delta 9 Medical Document which will act as a prescription for you to use medical cannabis. Make sure your doctor includes the daily dose in grams as well as the time period of the prescription (up to one year).

Step 3:
Complete the Delta 9 Registration Form
To finalize your registration and start ordering medical cannabis, you must complete our Patient Registration Form and mail us the original copy for final approval. Once your application has been verified, we’ll contact you so you can start ordering your medicine.
